Re-Entry Case Manager

The Institute for Human Services (IHS) is Hawaii's most comprehensive human services agency dedicated to ending and preventing homelessness. For over 45 years, we've been at the forefront of creating tailored solutions for those in crisis, offering a continuum of care that includes emergency shelters, housing programs, health services, employment assistance, and outreach.

The Re-Entry Case Manager provides direct case management support to paroles or consumers being released from incarceration as they face transition back into the community. This position provides case management services for reentry and coordinate housing for clients as they navigate their reentry or reintegration into housing, employment and the community. The Re-Entry Case manager will also work closely with the Re-entry Program Manager and provide progress reports of residents to the Re-entry Program Manager.

Compensation:

Up to $60,000 annually, actual pay commensurate with experience.

Essential Functions:

  1. Demonstrate excellent customer service consistent with the standards contained in the Vision, Mission, and Values of the organization.
  2. Provide direct service to clients through crisis intervention, counseling, case management, client grievance/dispute resolution, outreach, and other service activities as necessary.
  3. Work with Shelter management to screen any persons recently released clients entering into the shelter.
  4. Provide housing navigation services (gathering vital docs, applying for health insurance, SNAP and GA benefits etc.).
  5. Perform the primary functions of assessment, planning, facilitation, and advocacy through collaboration with the client’s probation officer, jail, work release, and other resources involved in the client's reentry.

Required Knowledge/Skills/Abilities:

  1. Knowledge of community resources for homeless, formerly incarcerated individuals with understanding of varying levels of care, as well as the standards for care within the community resources, mental illness and substance abuse and resources to address these issues.
  2. Strong communication and problem solving skills with ability to de-escalate and manage clients in crisis.
  3. Computer proficient in Microsoft applications, Google platform and other technology (tablet, apps) used to provide service to persons served.
  4. Possess personal qualities of integrity, patience and commitment to mission.
  5. Flexible and able to multitask; can work within an ambiguous, fast-moving environment.

Required Education and Experience

  1. Minimum high school diploma or equivalent and 1 year case management or outreach field experience preferably in a human services program serving disenfranchised populations.

Position Type/Expected Hours of Work:

Regular full time classification. General business hours, Monday through Friday, including some evenings, weekends and holidays. Hours and days may vary based on the needs of the organization.
