Housing Navigator

The Institute for Human Services (IHS) is Hawaii's most comprehensive human services agency dedicated to ending and preventing homelessness. For over 45 years, we've been at the forefront of creating tailored solutions for those in crisis, offering a continuum of care that includes emergency shelters, housing programs, health services, employment assistance, and outreach.

The Housing Navigator provides specialized case management and crisis support services for clients staying at the designated program. This position develops a housing plan, psychosocial assessment, Behavioral Health assessment and provide education and support for the client and others in the client’s significant support network. The Housing Navigator links clients to community resources to further independence and stability, working collaboratively with teams across the organization.

Compensation:

Starting from $20.00 hourly, actual pay commensurate with experience.

Essential Functions:

  1. Demonstrate excellent customer service consistent with the standards contained in the Vision, Mission, and Values of the organization.
  2. Manage a caseload and ensure the timely coordination of all CM services and linkages to substance abuse or mental health treatment, provide transportation to other involved or needed providers to coordinate member’s care and housing solutions.
  3. Provide supportive counseling and participate in crisis intervention services with individuals including mental status and risk assessment and linkage to crisis teams and hospitals.
  4. Provide on-going evaluation assistance, including Housing Assessments and ensure that documentation is upload into systems of record as required by the program;
  5. Maintains records of referrals, assessments and placement plans in compliance with HUD Housing Counseling Plan.

Required Knowledge/Skills/Abilities:

  1. Knowledge of housing processes and case management best practices.
  2. Strong communication and time management skills.
  3. Possess personal qualities of integrity, patience and commitment to mission.
  4. Flexible and able to multitask; can work within an ambiguous, fast-moving environment.

Required Education and Experience:

  1. High School Diploma or GED equivalent. Bachelor’s Degree in Human Services, Social Work or related field from an accredited university preferred.
  2. One to two years of direct case management or supportive Housing services experience working with homeless or other disenfranchised individuals.
  3. Valid Driver’s License and clean traffic abstract.

Position Type/Expected Hours of Work:

Regular Full-Time classification. General business hours, Monday through Friday, including some evenings, weekends and holidays. Hours and days may vary based on the needs of the organization.
